How to receive a vaccine fund?

To receive the vaccine fund, we ask you to fill out this form to access these funds.
We will need to have some basic information so we can help you book your vaccination such as; location, name, your disability status etc.

You can ask to have your vaccination either fully or partially funded. 

Before we can send funds, we will need to see either the booking confirmation or a receipt from after you've paid. We will then refund the full or partial cost of your vaccine.
