We made sure to make our form easy to fill out,
with short sections and will take roughly 3-5
minutes to complete.
We have also added an accessibility button so
you can view the form with your preferences
At the vaccine fund, we want to ensure that you are protected as we know we will be handling delicate information. Therefore please know that we have spent time
to assure you that your information is safe. Our JotForm is encrypted, and will not be stored for longer than it needs to be. Additionally, everything else will be handled on secure networks. Only one person will be dealing with the transactions and if you do run into any issues, you can always email us.
